*Registration Notes*

First time users will need to make a new account, this new program does not link to Family ID. This will be a Family Account - you will be able to register multiple students. 

Auto-Fill is not accepted, please manually type the information during the registration process, otherwise the lines will be red and you will not be able to submit. 

*Physical Examination News (Updated June 2024)*

Students registering for Athletics/Activities must submit a physician signed/stamped hard copy of the Preparticipation Physical Evaluation Medical Eligibility Form AND a copy of the Preparticipation Physical to the Health Office.

You may scan and email these forms to the School Nurse but photos and individual photos of each page will not be accepted.

The copy of the physical will not be used to determine clearance; it is to be recorded on the A-45 pursuant to N.J.A.C. 6A:16-2.4 and N.J.S.A. 18A:40-4. 

Once your Eligibility Form is reviewed and approved by the School Physician, final clearance will be issued by the School Nurse. 

The physical must be within 365 days of the first practice. In accordance with N.J.S.A. 18A:40-41.7, if a student’s physical was completed more than 90 days prior to the first day of practice, a Health History Update Questionnaire is required to be submitted to the School Nurse.
